Requesting Immunization Documents

The FIU Student Health Services stores Immunization documents for a time frame of seven (7) years.

Please complete an FIU Authorization for Release of Immunization Information Form prior to requesting a record. FIU currently charges $5.00 for an official FIU record or a copy of your original record. After payment is confirmed, the Health Compliance Unit can fax or mail Immunization records as requested
